The Take Charge program offers birth control methods and family planning services to low income women and men to prevent unintended pregnancy.

 

The most comprehensive Family Planning Coverage of any insurance company in Washington State.
Since implementation on July 2001, 100,000+  men and women successfully enrolled in this 5 year demonstration waiver. Eligible persons receive  pre-pregnancy family planning services to help them plan; if or when to have children, and the timing and spacing of pregnancies. Applications for TAKE CHARGE family planning services are available hee at the clinic.

TAKE CHARGE OFFERS:

  • Gynecological exam from a TAKE CHARGE (TC) provider 

  • One session of application assistance per year

  • One initial education, counseling, and risk reduction (ECRR) service

  • One follow-up ECRR service every 10 months for women, annually for men

  • Prescription birth control methods from a TC provider or any pharmacy that accepts a TAKE CHARGE card;

    • Emergency contraception pills including Preven and Plan B

    • Patch or Nuva-Ring 

    • Diaphragm or cervical cap

    • Depo Provera 3-month shot

    • IUD including Mirena

    • Birth control pills

  • Medications directly related to a birth control method

  • Tubal ligation from a physician (a TC or Ancillary Service Provider) that accepts a TAKE CHARGE card

  • Vasectomy from a physician (a TC or Ancillary Service Provider) that accepts a TAKE CHARGE card

  • Natural family planning and abstinence education

  • Over-the-counter (OTC) Food and Drug Administration (FDA) approved contraceptives without a prescription from any pharmacy that accepts a TAKE CHARGE card; i.e. male and female condoms, contraceptive foam, gel, cream, jelly and suppositories.

  • Vaginal contraceptive film may be obtained at a family planning clinic

  • Treatment of STD when performed in conjunction with a family planning diagnosis and medically required as part of the client's selected birth control method

Please note : not all OTC products can be purchased with a TAKE CHARGE card. Amounts will vary depending on pharmacy/clinic stock.

To qualify, you must:

. Have income at or below 200% of the Federal Poverty level. For example: $18,620 for a single person and $37,700 a year for a family of four. (see chart above)


. Be a U.S. citizen or have a green card


. Be a resident of Washington State


. High school and college students DO NOT count parental income, so all high school students and most college students will qualify
